  1. Structure-process-outcome: a causal model for quality in nursing homes

    Sainfort, François; Ferreira, Pedro Lopes
In this paper, we explore the correlation structure between some indicators of quality in nursing homes. These indicators include Structure (Staff and Faculty), Process (Social Care, Medical Care, and Resident involvement), and Outcome indicators (Medical Outcome, Social Outcome, and Organizational Performance). Using path analysis, a causal model is also hypothesized and empirically tested based on these indicators and on data collected for 104 nursing homes in Wisconsin. This causal model constitutes a basis for formulating quality improvement strategies.

  2. Stability of patient perception of quality of hospital care over time

    Ferreira, Pedro Lopes
The purposes of this study were to determine patient attitudes towards the quality of Hospital care, and to assess whether those attitudes remained stable over time. Theoretical constructs to measure patient satisfaction with hospital care were tested. From among the family of models available, the multiattribute utility model (MAU) was chosen. This approach employs a structured list of attributes to which patients respond by indicating relative importance (weight) and utility of each attribute. Patients diagnosed with myocardial infarction were selected from nine hospitals. Through self-administered questionnaires, using tradition Likert-type scales, data were collected from 164 patients at two points in...

    Ferreira, Pedro Lopes

  4. Family of instruments to evaluate hospital quality: a pilot study

    Ferreira, Pedro Lopes
Quality assurance in health care is usually disconnected from the object of its study. Even when quality assessment belongs to the day-to-day hospital management, it is seldom integrated in the decisions, vital pieces of the hospital services management. This situation would be unacceptable in other industries. In this study we focus our attention on how to obtain knowledge about the way customers - patients, physicians, and nurses - evaluate the quality of hospitals viewed as networks of interrelated processes and systems. The purpose of these instruments to assess hospital quality is to monitor hospital quality trends of the major processes...

Ferreira, Pedro Lopes; Rosete, Marta Luísa; Raposo, Victor

  6. The Portuguese Version of the EORTC QLQ-C30

    Ferreira, Pedro Lopes
In this paper we report the results of a study aimed at translating and culturally validating the quality of life instrument QLQ-C30 initially developed by the European Organization for Research and Treatment of Cancer (EORTC). We begin by mentioning the forward-backward translations process used to construct a Portuguese version semantically equivalent to the original version. After minor revisions, the instrument was pilot tested in fifteen patients with breast cancer. A brief description of the results of the pilot test with women with breast cancer and with other two studies (mastectomized women and women with lymphomas) is presented in this paper...

    Ferreira, Pedro Lopes; Raposo, Victor

  8. The impact of non-monetary factors on the primary care utilization in Portugal. Finite mixture models applied to on-site and truncated samples

Lourenço, Óscar Domingos; Ferreira, Pedro Lopes
In the Portuguese National Health Service (NHS) patients have to pay a co-payment of 2€ to visit a GP in the health centres. Therefore, the monetary price associated to each visit is low and, with a high probability, is not a factor that affects the utilization of consultations in health centres. On the other hand, in any health system in which the monetary cost to consume medical care is very low other kind of costs can emerge as determinants of medical care utilization. The Portuguese NHS suffers from several time-related inefficiencies and so, the non-monetary form of co-payment is a...

  9. Policy assessment based on subjective data: a new challenge to policy makers and researchers

Ferreira, Pedro Lopes; Lourenço, Óscar Domingos

  10. Econometric analysis of healthcare utilization : an alternative hurdle specification using latent class models

Lourenço, Óscar Domingos; Ferreira, Pedro Lopes; Barros, Pedro Pita

  11. Modeling health state preference data using fixed and random effects models

Ferreira, Lara Noronha; Ferreira, Pedro Lopes; Pereira, Luís Nobre
The use of preference-based measures of health in the measurement of Health Related Quality of Life has become widely used in health economics. Hence, the development of preference-based measures of health has been a major concern for researchers throughout the world. This study aims to model health state preference data using a new preference-based measure of health (the SF- 6D) and to suggest alternative models for predicting health state utilities using fixed and random effects models. It also seeks to investigate the problems found in the SF-6D and to suggest eventual changes to it.
