A case of chronic constrictive pericarditis of probable tuberculous origin in a middle-aged white male is discussed. The localization of calcium was unusual in that it was confined to a thick band encircling the ventricles and extending into adjacent myocardium 2 cm caudal to the atrioventricular groove. The pathogenesis, clinical and surgical indications for operation, and the prognosis are considered, together with a discussion of operative techniques employed.

USE OF A NEW POLYBUTILATE-COATED POLYESTER SUTURE IN CARDIOVASCULAR SURGERY - Reul, George J.
Polybutilate coated polyester (Dacron) sutures were evaluated in a variety of cardiovascular procedures performed on 117 patients. The age range was from 6 months to 88 years, and long-term follow-up was available in 100 patients. The most frequently sutured tissues were those of the aorta, atrium, fascia and pericardium. Suture sizes ranged from 1-0 through 5-0, the most common being 3-0. There were no suture related complications, and the performance was found to be superior to non-coated polyester sutures and other forms of coated sutures. An operative procedure is described for repair of transposition of the great arteries in which the left ventricle is the â??aorticâ?? ventricle and the right ventricle is the â??pulmonicâ?? ventricle. The technical feasibility of this procedure is supported by its successful application in experimental animals. A modified procedure, which may be applicable in certain complex forms of transposition of the great arteries and other perplexing anomalies for which there are no known repairs, is discussed.

In an attempt to clarify some of the controversy regarding the concepts of myocardial ischemia, various cardiac pathologies are presented to demonstrate several of the unusual mechanisms involved in the causes of transient ischemic events of the myocardium. A review of the literature is presented, with a discussion of the current approach to managing these unusual clinical conditions.

A patient with severe pulmonary hypertension and no evidence of right ventricular failure who had a 4 mm â??aâ?? dip on the pulmonic valve echocardiogram is reported. Although other echocardiographic abnormalities suggesting pulmonary hypertension were recorded in our patient, the normal â??aâ?? dip of the pulmonic valve in the absence of right ventricular failure appears to be an exception to previously reported findings. We suggest motion of the entire pulmonary artery as an explanation for this phenomenon.

A large pulmonary arteriovenous fistula was discovered in a patient with long-standing cyanosis, clubbing and dyspnea, with no other cardiovascular signs or symptoms and a normal chest roentgenogram at the time of cardiac catheterization and pulmonary angiography. The fistula was overshadowed by the cardiac silhouette. Surgical resection was successful. Although rarely undetected on the chest roentgenogram, this potentially lethal malformation should be considered in the differential diagnosis of cyanosis unaccompanied by other cardiovascular signs or symptoms.
