  1. Endoscopic ultrasonography features of gastric mucosal cobblestone-like changes from a proton-pump inhibitor

    A 68-year-old man with no symptoms presented to Hokkaido University Hospital for esophagogastroduodenoscopy screening. He had a history of Helicobacter pylori eradication. Initial esophagogastroduodenoscopy showed no gastric cobblestone-like mucosa or gastric cracked mucosa. After 1 year, he received esomeprazole (20 mg) once daily for heartburn at another hospital. Esophagogastroduodenoscopy was performed after 2 years of esomeprazole administration.     In our new echocardiographic method, pulmonary regurgitant velocity immediately before right atrial (RA) contraction is used to estimate pulmonary artery diastolic pressure (PADP) and mean PA wedge pressure (MPAWP). Our aim here was to compare the usefulness of this new method with that of the conventional method, which uses pulmonary regurgitant velocity at end diastole.     Background: Genetic variants responsible for childhood interstitial lung disease (chILD) have not been studied extensively in Japanese patients. Methods: The study population consisted of 62 Japanese chILD patients. Twenty-one and four patients had pulmonary hypertension resistant to treatment (PH) and hypothyroidism, respectively. Analyses of genetic variants were performed in all 62 patients for SFTPC and ABCA3, in all 21 PH patients for FOXF1, and in a limited number of patients for NKX2.1.     For several decades, the neurotoxicities of anesthetics to the developing brain have been reported by many researchers focusing on various phenomena such as apoptosis, neurodegeneration, electrophysiological aberrations, and behavioral abnormalities. According to these reports, signals via N-methyl-D-aspartate receptors (NMDA-r) and/or γ-aminobutyric acid type A receptors (GABA(A)-r) are implicated in the anesthetic neurotoxicity.   11. 北海道HIV福祉サービスネットワークの構築

    目的:HIV 陽性者へ困難なく福祉サービス提供を行うため,HIV 陽性者からの利用申し込みを拒否しない福祉サービス事業所が参加するネットワークを構築する。方法:北海道ブロック各拠点病院HIV 担当ソーシャルワーカーにより以下の福祉事業所へネットワークへの参加依頼を行った。(1) 2012 年にはばたき福祉事業団が道内の福祉施設を対象に行った「施設を対象としたエイズ患者/HIV 感染者生活支援実態調査」においてHIV 感染者の受け入れ可能と回答した事業所。(2) HIV 陽性者へのサービス提供の経験がある事業所。(3) 北海道大学病院が行っているHIV 出張研修を受けた事業所。(4) ソーシャルワーカーが職能団体を通じて繋がりのある事業所。さらに,北海道保健福祉部から北海道内の社会福祉法人に対して,ネットワーク参加依頼の文書が配布された。結果:ネットワークへの参加を働きかけたところ,51 件の法人本部,福祉事業所代表の登録が得られ,グループ内の407 件の福祉事業所に利用申し込みをすることが可能となった。一方でネットワークへの参加を断った福祉事業所もあった。考察:行政からの依頼で多くの社会福祉法人のネットワーク登録があったことから,ネットワーク構築には行政との連携が有効であると考えられた。現在,ネットワークに参加している事業所は札幌圏に集中している。また,訪問看護サービスが多く参加している一方で入所サービスの参加が少ないという偏りもある。HIV 陽性者のニーズに応じた福祉サービスを提供するためには,ネットワークのさらなる拡大が必要である。

    Anti-vascular endothelial growth factor (VEGF) therapy has demonstrated efficacy in treating human metastatic cancers, but therapeutic resistance is a practical limitation and most tumors eventually become unresponsive. To identify microenvironmental factors underlying the resistance of cancer to antiangiogenesis therapy, we conducted genomic analyses of intraperitoneal ovarian tumors in which adaptive resistance to anti-VEGF therapy (B20 antibody) developed.     The intestinal microbial ecosystem is actively regulated by Paneth cell-derived antimicrobial peptides such as α-defensins. Various disorders, including graft-versus-host disease (GVHD), disrupt Paneth cell functions, resulting in unfavorably altered intestinal microbiota (dysbiosis), which further accelerates the underlying diseases.
