Objective: Trans-sphenoidal surgery is often combined with other approaches for the treatment of middle cranial base tumors. By combining a maxillotomy with trans-sphenoidal approach, significantly wider exposure to these regions is gained. However, endoscope-assisted techniques have also been used for sellar and parasellar and upper clival regions. Chronic otitis media, unlike trauma, seldom leads to the development of tension pneumocephalus. Rarely, it occurs as a complication of mastoid surgery. A Melanesian woman sought treatment for loss of speech, hemiparesis, and headache 4 weeks after undergoing canal-down mastoidectomy for cholesteatoma. The tension pneumocephalus was decompressed urgently by aspirating air via a cannula through the burr hole, and the mastoid cavity was obliterated. She recovered completely. Twelve additional patients with postmastoidectomy pneumocephalus reported in the literature are reviewed.

This study was conducted to identify clinical and histologic factors that would influence, independently of tumor staging, postoperative facial function after removal of a vestibular schwannoma. A prospective study was performed on 35 consecutive patients with vestibular schwannomas who underwent the translabyrinthine approach. Facial function was assessed before and 1 year after surgery. The factors that influenced the postoperative outcome of the facial function independently of tumor staging were the absence or the desynchronization of homolateral auditory brainstem responses and tumor edema. The aim of this investigation was to evaluate the suitability and usefulness of the Stealth Station intraoperative guiding system (Medtronic Sofamor Danek, Memphis, TN) in a variety of indications. Eleven intraoperative imageguided procedures were performed for anterior or lateral skull base lesions. The most common neurosurgical approaches included frontal, coronal, and parietotemporal access. Neuronavigation reliably allowed the extent of tumor configuration and risk zones (e.g., blood vessels) to be visualized. The site of origin, projection, and relationship of aneurysms arising from the ophthalmic segment of the internal carotid artery (ICA) to adjacent structures are heterogeneous. Based on a retrospective analysis of 61 patients with aneurysms in this location, we developed a simple numerical classification system to guide surgical planning. Most meningiomas are slowgrowing tumors that do not rapidly recur after subtotal removal. After subtotal resection of a meningioma a 47yearold woman developed a large extracranial recurrence 1 year later. The recurrence was resected successfully. On histological examination the typical characteristics of a meningioma were absent. Based on immunohistological and ultrastructural studies, the tumor was classified as a grade III meningioma of the newly recognized rhabdoid subtype.

This study compared mandibular distraction and vertical ramus osteotomy in terms of their effectiveness at increasing access to the cranial base and distal internal carotid artery. Cerebrospinal fluid (CSF) rhinorrhea is a common condition managed by most otolaryngologists with the help of nasal endoscopy (sinoscopy). In the last 2 years, we have used a neuroendoscope with a working sheath to treat nine patients with CSF rhinorrhea. One patient developed a recurrence 1 month after treatment but then responded to conservative treatment. We conclude that the treatment of CSF rhinorrhea by a neuroendoscope with a working sheath is safe, effective, and easy and obviates the need for a separate sinoscope.

Sarcomas of the skull base occurring in the field of prior radiotherapy are rare; only a few have been reported in the English literature. We report a fatal osteosarcoma of the skull base arising in a patient with McCuneAlbright syndrome and severe fibrous dysplasia of the skull base 22 years after radiation therapy for a pituitary adenoma. The choice of surgical approach to tumors of the cranial base in children is determined by strict criteria. The criteria include optimal visibility for the surgeon, minimal possible impairment of facial skull growth, and the preservation of motor and sensory nerve integrity. Cavernous hemangiomas of the cavernous sinus belong to a welldistinguished entity of extraaxial cavernous hemangiomas located in the cavernous sinus and have a typical appearance on magnetic resonance imaging (MRI). Severe intraoperative bleeding has been described during the excision of these lesions that is probably associated with their pathological features. An atypical case of a sclerosing lesion with distinct MRI features is described. In these particular cases, especially with giant hemangiomas where en bloc excision would be difficult, safe internal decompression and resection can be achieved.
