    O modelo de saúde mental comunitária implica uma perspectiva bio‑psico‑social das questões da saúde/doença mental, assim como um conjunto de valores que preconizam a equidade no acesso aos serviços, o tratamento na comunidade, o respeito dos direitos humanos, uma visão de recuperação, a promoção da vida independente e da integração social e a participação de utentes e famílias. De acordo com as prioridades definidas pela União Europeia, os serviços de saúde mental devem garantir a aplicação destes princípios, e promover a prevenção, o tratamento, a reabilitação e a promoção da saúde mental.   2. A psychological and communitarian perspective of early attention to psychoses

    The authors discuss the characteristics and the preliminary findings of the Prevention in Mental Health ‑ EAPPP (Early Detection & Attention of Patients with Risk of Psychosis), a team inserted in the public Primary Health Care System of Barcelona, Spain. Special attention is given to the four core domains of the team work: selection of target populations, the system for detection of first episode psychosis (FEP) and at‑risk mental state (ARMS) subjects in the community, the close relationship with the social and educational systems, and the psychoanalytic components of the inter‑paradigmatic framework. The community focused work and the psychological and inter‑paradigmatic framework are core differential components of the EAPPP unit in the field of EPA (early psychosis attention).

    En el presente texto se analiza el impacto que en el ámbito de la salud mental comunitaria tuvo en Galicia la aprobación del Plan Estratégico de Salud Mental 2006‑2011 y su desarrollo en los tres primeros años de aplicación. El trabajo comienza con una contextualización histórica de la nueva política en salud mental que sostiene la experiencia que se describe posteriormente, haciendo hincapié tanto en el desarrollo de los servicios como en los cambios habidos en la praxis asistencial con la puesta en marcha de diversos programas. El artículo termina con una reflexión sobre las incertidumbres del futuro debidas a los avatares políticos.

    Italian psychiatry has gained International attention after its radical reform of 1978, which established the progressive closure of mental hospitals and the establishment of community services throughout the country. However it is technically inappropriate to talk about Italian psychiatry as the devolution process has transferred to the regions all competences about policy, planning and evaluating health services. This explains the variety of “community psychiatries” that can be found along the peninsula and the reasons of interest that can arise from their comparison. Such research is beset with difficulties including fixing models long enough to get clear comparisons, the absence of consistency in description (particularly of comparator services) and the inevitable contamination from the ‘Pioneer’ effect of highly motivated teams.
