Leber's hereditary optic neuropathy: case report and literature review
- Teive,Hélio Afonso Ghizoni; Troiano,André Ribeiro; Raskin,Salmo; Werneck,Lineu César
CONTEXT: Leber's hereditary optic neuropathy is an important cause of progressive painless visual loss among young male patients. OBJECTIVE: To report on a case of a young patient with a clinical and neurophysiological condition suggestive of Leber's hereditary optic neuropathy, confirmed by genetic testing. CASE REPORT: We describe a 17-year-old male with progressive bilateral visual loss. Two maternal uncles had had similar patterns of visual loss. The patient had a history of smoking and alcohol abuse. Neuro-ophthalmological examination revealed visual acuity of 20/800 in both eyes, with decreased direct and consensual pupillary light reflexes. Fundus examination demonstrated pale optic discs....

Leber's hereditary optic neuropathy is a mitochondrial hereditary optic neuropathy that usually affects young males but may occur at any age and in males or females. The clinical features are usually acute bilateral simultaneous or sequential visual loss with a central acuity and central visual field loss. Mitochondrial DNA testing of the blood may demonstrate the mutation. Because the disease is transmitted via mitochondria, affected females pass the mutation to all of their offspring, while affected males cannot transmit the disorder. Patients may exhibit peripapillary telangiectasias, and the disc usually has an acute appearance of pseudoedema. Although vitamin supplementation has...

Fiber optic cable assemblies for space flight II:
- Thermal And Radiation; Melanie N. Ott
Goddard Space Flight Center is conducting a search for space flight worthy fiber optic cable assemblies that will benefit all projects at all of the NASA centers. This paper is number two in a series of papers being issued as a result of this task to define and qualify space grade fiber optic cable assemblies. Though to qualify and use a fiber optic cable in space requires treatment of the cable assembly as a system, it is very important to understand the design and behavior of its parts. This paper addresses that need, providing information on cable components shrinkage testing...

Several Primary Mutations result in Leber's hereditary optic neuropathy, including mitochondrial deletions at positions 11778, 3460, and 14484. Although the 11778 is the most common mutation, the 11484 has the best prognosis for spontaneous recovery. This case exhibits the 3460 mutation.

This 35-year-old otherwise-healthy woman developed typical optic neuritis OD with excellent recovery. She had no clinical evidence of multiple sclerosis at that time. She presented in August of 1991, at which time perivenous sheathing was seen in the retinal periphery OU. A limited workup was negative, including chest x-ray, ANA, ACE, and FTA-ABS. In the 3 subsequent years, she has not developed any clinical evidence of multiple sclerosis. Pair with 95_07, 95_08, and 95_09.

What is the prognosis of optic neuritis? How often does it lead to multiple sclerosis?
- Sanders, Sharon; Del Mar, Chris; Purdy, Sarah; Spinks, Annelise; Tait, Lisa; McAvoy, Brian
This rapid review of the literature tried to answer the question of the prognosis of optic neuritis leading to multiple sclerosis (MS). The risk of developing MS after a single episode of optic neuritis ranged from 13% to 58%.
